International Internship/Practicum, Individual Study Forms

Please complete the following forms if you plan to carry out an internship, practicum or individual study abroad while enrolled in Colorado State University credit. If credit is granted through an outside institution (such as a third party provider or foreign institution), please contact the Study Abroad Office.

Please answer the following questions to determine if you are required to fill out the Internships/Practicum forms.

  1. [YesNo] Does this experience fulfill a requirement for a CSU program of study?
  2. [YesNo] Are you receiving any funding through CSU for this experience (i.e. the program is funded through a grant awarded to a CSU faculty member, department, or college; you are receiving a scholarship, assistantship, or are on CSU payroll)?
  3. [YesNo] Will you be enrolled for credit (including internship credit or independent study credit) while you are abroad?
  4. [YesNo] Are you accompanying a CSU employee or affiliate on official business?
  5. [YesNo] Would potential participants or family members perceive this program as being sponsored by CSU?
  6. [YesNo] Has the program been represented as a CSU-affiliated activity?
  7. [YesNo] Is it featured on a CSU webpage?
  8. [YesNo] Has it been marketed to students by a CSU employee?

If you answer “yes” to ANY of the questions above, you are required to register your experience abroad with International Education/Office of International Programs.
