Pesticide Record Book for Private Applicators

The following forms and instructions were created to provide private applicators with a simple method of meeting the federal record keeping requirements for restricted use pesticides (RUPs). These materials were modified from the Pesticide Record Book for Private Applicators shown above. The forms available for download below are in a whole page format that can fit in a three-ring binder or filed. The smaller, pocket-sized book is still available.
